The Geriatric Care Manager
A Geriatric Care Manager (GCM) is a trained professional employed by Senior Outreach Services to assist older individuals and their families access a range of necessary services. Care Managers are trained in gerontology, social work, nursing, or counseling.
Senior Placement and Referral
Free services to help families to help family members find a comfortable, secure, and caring environment for loved ones and offer ongoing professional services after placement.
