Home Safety

Home Safety Assessment and Modification Services

The Fall Prevention Program of Contra Costa County offers FREE Home Safety Assessment and Modification services for those that qualify.

. Are you an unpaid Care Provider that assists an older adult that is at risk of falling down?

. Would you both benefit from an in-home fall risk assessment?

. Would grab bars or other safety devices assist you in caring for your client?

. Is your client over 60 years old?

. Is your client a resident of Contra Costa County?

If you meet these criteria and would like to receive assistance through the Home Safety Program, please complete these steps:

1.) Open this link: Fall Prevention Home Safety Referral Form.

2.) Print and fill out the form.

3.) Fax the completed form to: Gennifer Mountain, Program Manager at (925) 946-1869.

Alternatively, please call us at (925) 937-8311 ext. 110 or e-mail Gennifer at gmountain@mowsos.org so we may answer any questions you may have.
