- Determination of eligibility for assistance
- Long-term planning and placement
- Home health and personal care aid

- Fiduciary and Durable Power of Attorney
- Counseling and crisis intervention
- Information and referral
- Meal delivery
- Coordination of Doctor's appointments
- Errand services
- Handyman Services (lawn maintenance/ small home repairs)
- Hiring/screening
- Caregiver coordination
- Consultation at home or in a hospital or nursing home
- Guidance during the decision making process based on individual preferences and needs
- Personally escort family members to recommended facilities
- Negotiate best price for services at the chosen facility
- Help complete all paper work
- Follow-up after placement
- Offer LVN monitoring of the facility or quality of care, if needed
